Assessments of each child or adolescent served include information on his or her:

  1. Developmental history, such as developmental age factors, motor development, and functioning.
  2. Medical or physical health history
  3. Culture/ethnicity
  4. Treatment history
  5. School history
  6. Language functioning, including:
    1. Speech functioning
    2. Hearing functioning
  7. Visual functioning
  8. Immunization record
  9. Learning ability
  10. Intellectual functioning
  11. Family relationships
  12. Interactions with peers
  13. Environmental surroundings
  14. Prenatal exposure to alcohol, tobacco, or other substances
