2.C.1.a-f.

A written person-centered plan is:

  1. Developed with:
    1. The active participation of the person served
    2. The involvement of family/legal guardian of the person served, when applicable and permitted
  2. Prepared using the information from the assessment process
  3. Based upon the person’s:

    2. Needs
    3. Abilities
